Regarding Alternative Vaccination Option for Those Who Have Not Received the DTaP-IPV (Four-in-One) Vaccine

For residents of Minato City who have not yet received the DTaP-IPV (four-in-one) vaccine, an alternative option using the DTaP-IPV-Hib (five-in-one) vaccine is now available.You may continue to use your existing vaccination questionnaire form (for the four-in-one or three-in-one vaccine), so please be sure to bring it with you.
